Hospice Care Conditions
Patients who have been diagnosed by their physician to have a life-limiting disease or condition with an estimated six months or less to live (if the disease process continues its natural course) can qualify for hospice care. Hospice care is provided in the patient’s private residence, skilled nursing facility/nursing home or assisted living community.
